21st CCLC Profile and Performance
The reporting of teacher survey results constitutes one section of the 21st CCLC Annual Performance Report (APR), which all 21st CCLC grantees are required to complete once a year in accordance with federal reporting requirements. The purpose of the teacher survey is to ask school-day teachers to report on the extent to which certain behaviors exhibited by a center's regular attendees improved or did not improve during the APR reporting period.
You can download a form-fillable, Word document containing the teacher survey to be completed as part of the 2004 APR process. If you are prompted to either open or save the document, please save it to a location where you can find it later.
When you open the document, a box should come up asking if you want to enable the macros associated with the document. Please click "Enable Macros" in order to use the full functionality of the document. If "Enable Macros" is grayed out, then you must check the "Always trust macros from this source" checkbox first.
When you have completed the survey, you should forward it to your 21st CCLC contact in accordance with the instructions they provided to you. If you are sending your completed survey electronically to your 21st CCLC contact, please remember to save the completed survey to either your desktop or a network folder.
